   Print the Orange County Application for Examination/Employment form (4 pages).
2. Complete Sections 2, 3, 4, 6 (if needed), 7, 8, 9, 10, 11, 12, 13, and 14.
   • In the Remarks Box (on the last page):
     • Write which program you’re applying for (e.g., park program, football, cheerleading, track, tennis, basketball, lacrosse, or soccer).
     • List 3 references with their phone numbers.
3.  Sign and date the form at the bottom.

Submit Your Application.
Do not mail it to the Goshen address!

Eligibility Requirements

• You must be at least 15 years old to apply.
• If you’re under 18, you must provide your working papers.
